These forms are for injured workers. If you’re an employer looking for a form, contact your WorkCover insurer directly — the forms below won’t apply to you.

Initial WorkCover claim form
Starts your WorkCover claim — for most people, this is the only way a claim can begin. Complete it, sign it, and lodge it with your employer or insurer.

Conciliation form
Use this to request conciliation when you want to contest a decision your WorkCover insurer has made — a required first step before most disputes go further.

Certificate of capacity
Needed to keep claiming weekly payments if your injury affects your ability to work. Give this to your doctor or treating practitioner to complete and sign.

Impairment claim form
Starts a lump-sum claim for permanent impairment. You can lodge it yourself, but we’d recommend getting legal advice first.

For travel and medical treatment costs. Use this to get money back from your WorkCover insurer for expenses related to your injury.

E-signatures are usually fine
Most insurers accept a form filled in and signed electronically — a few still ask for a wet signature, so check if you’re unsure.

You can also get copies directly from WorkSafe, your WorkCover insurer, or — for the initial claim form — your local post office.
